Frequently Asked Questions
What is hormone replacement therapy?
Hormone replacement therapy (HRT) uses medication to supplement hormones the body is making less of — for example, estrogen (with or without a progestogen) to treat certain menopause symptoms, or testosterone for men with confirmed testosterone deficiency. Depending on the hormone and your needs, it may come as a pill, patch, gel, cream, vaginal product, or injection.
What is the difference between HRT and BHRT?
"Bioidentical" (BHRT) describes hormones that are chemically identical to those the body produces. Several FDA-approved hormone therapies contain bioidentical hormones, such as estradiol and micronized progesterone.
Compounded bioidentical hormones are custom-mixed by pharmacies and are not FDA-approved, meaning they haven't been reviewed by the FDA for safety, effectiveness, or consistent dosing. They have not been shown to be safer or more effective than FDA-approved options. Your physician can explain the differences and which options may be appropriate for you.
How do I know whether hormone therapy is appropriate for me?
Only a medical evaluation can determine that. Your physician will consider your symptoms, age and life stage, medical and family history, current medications, and risk factors — such as a history of certain cancers, blood clots, stroke, heart disease, or liver disease — along with lab results when relevant. Hormone therapy isn't right for everyone, and other approaches may be recommended.
What happens during my initial consultation?
We'll review your health history, symptoms, medications, and goals; perform an examination if appropriate; discuss whether lab testing is needed; and talk through options — hormonal and non-hormonal — with time for your questions.
Are laboratory tests required?
It depends. Diagnosing testosterone deficiency in men requires laboratory confirmation, usually with repeat morning blood tests. For menopause care, decisions are often based on age and symptoms, though your physician may order tests to rule out other causes or to support safe treatment. Ongoing therapy may also call for periodic labs.
What are the potential benefits and risks of hormone therapy?
For many appropriate candidates, menopausal hormone therapy is an effective treatment for hot flashes and night sweats and can help genitourinary symptoms. For men with confirmed deficiency, testosterone therapy may help related symptoms. Individual responses vary.
No treatment is risk-free. Depending on the therapy, risks can include blood clots, stroke, gallbladder disease, and — with some regimens and durations — breast cancer for women; and for men, increased red blood cell counts, acne, fluid retention, and reduced fertility. Your individual risks depend on your age, health history, the type of therapy, and how long it's used, and will be reviewed with you in detail.
How is hormone therapy monitored?
Through scheduled follow-up visits to review symptoms, side effects, and blood pressure, plus lab tests when appropriate — for men on testosterone, this typically includes hormone levels and blood counts. Treatment plans are re-evaluated regularly, including whether to continue, adjust, or stop therapy, and routine health screenings are kept up to date.
